Commission Circular No: 640

18th August, 1995

Vice-Chancellors of Universities
Directors of Institutes.
Directors of Affliated University Colleges

NOMINATION FOR THE UNIVERSITIES PROVIDENT FUND

Universities Act No.16 of 1978 as last amended by Universities (Amendment) Act 1 of 1995 has made provisions for nominations to the Universities Provident Fund. Every contributor is required to nominate a person to whom the monies lying to the credit of the contributor's account in the Provident Fund shall be paid upon the death of such contributor. Hence, Registrar/Senor Assistant Registrar of every Higher Educational Institution should make arrangements to ensure that they obtain the relevant nominations from every employee and forward them to the University Grants Commission.

If any employee declines to nominate a person/s, he or she may return the form with the endorsement
"NO NOMINATION"
with his/her signature. If the contributor dies without valid nominations, UGC will pay the proceeds to the persons lawfully entitled to such payment.

Since all employees of the Higher Educational Institutions are required to submit their nominations, the following procedure may be followed:

(1) Regarding the employees who are already in service, the respective nominations should be obtained immediately and be forwarded to the UGC with a list, arranged in UPF number order. You may keep a photo copy of the nomination form in the personal file of the employee.

(2) In case of new recruits, every employee should be given the relevant form at the time of issuing the letter of appointment and should be requested to return it along with the letter of acceptance.

(3) When New Numbers are requested, all the relevant nomination forms should be sent to the UGC with such request. New UPF Numbers will not be issued without the nomination forms.

(4) All nomination forms for those who are in service as at 1st of August 1995 should reach the UGC office on or before 30th September 1995.

Kindly note that forms issued by the UGC should be used to submit Nominations. These forms are available in all three languages. The number of Forms required for your Institute/Institution, in the desired language, can be obtained from the Assistant Accountant, UPF Section of the University Grants Commission on written request.

All Heads of Departments may be kindly requested to exercise due care in certifying the signatures of the employees to ensure that the nominations are being correctly done.

Separate instructions and guidelines are provided with the nomination forms in order to assist the contributors to fill the forms accurately.

Kindly comply with these instructions/guidelines.
